Two ways to apply for DMVNow services are in-person and by mail.

Apply In-Person

  • If you prefer to apply in person, you can visit a customer service center at the DMVNow. 
  • You must fill out either the Virginia Birth Certificate Form (DL 81) or the Virginia Vital Record Form (DL 82), depending on the type of certificate you require. 
  • You must also provide a valid photo ID and pay a fee of $14 for a record search. 
  • It is important to note that these fees apply regardless of whether or not the DMV finds the record.

Apply By Mail

  • If you prefer to apply by mail, you can send your Virginia Vital Records application directly to the Virginia Department of Health Department of Vital Records at PO Box 1000, Richmond, Virginia 23218. 
  • You must include a copy of your identification and a check or money order payable upon request. Your request must also include the Condition Health service.
